Office Information and Protocols

Appointments:

Appointments are made with the receptionists. Individual problems require different lengths of time to evaluate and treat you properly. It is important that you accurately describe your problem so that the appropriate amount of time can be scheduled. Please let the receptionist know if you are coming in for more than one problem. We request that new patients have a comprehensive history and physical exam at their first appointment or soon thereafter. This will allow the doctor to become better acquainted with you, to review your family’s medical history, to identify current health problems, and to include you in the development of a plan for your health. The new patient comprehensive physical exam is generally about 1 hour and may include laboratory tests. Please try to schedule routine or check-up appointments several weeks in advance to get the day and time you desire.
 If you find it necessary to cancel an appointment, please do so 24 hours prior to your scheduled appointment time or you may incur a cancellation fee. We understand that there are always exceptions and that patients also experience emergencies. If you miss your appointment without having cancelled your scheduled time, it will be considered a “no show”. Habitually missing appointments without canceling may result in the termination of the patient/physician relationship.

Extended Care Coverage:

If you or a family member requires care in an extended care facility, we will be happy to discuss with you the facilities in the area that we service.

Illness/Questions for the doctor:

For routine calls, a message will be taken and the call returned later in the day by the physician or the nurse. For more acute problems, please explain the urgency to the receptionist. She will notify the doctor who will return your call as soon as possible. In either case, it is necessary to leave your message with our receptionist. You may also ask to speak to the nurse who is available for urgent questions. For an acute illness or injury, calling earlier in the day will allow us to see you that same day. Please try to keep your phone line free for the return call, have your pharmacy phone number ready in case medicine needs to be prescribed, and have a pen and paper to write down any instructions.

Non-Office Hours Emergencies:

Our telephones are answered by a 24 hours answering service when the office is closed. They will take your message and notify the doctor on call who will return your call as soon as possible. For extreme emergencies, dial 911. They will contact us after the situation is stabilized. You should keep the office number and the number of poison control near your telephone for family members and babysitters. No narcotics or controlled substances will be filled after hours. This is not the time to call for routine refills or follow up questions. Antibiotics will only be given after hours in extreme cases which do not allow time for an office visit to be scheduled.

Lab Results:

We will notify you regarding your lab results either by mail or phone call. Please do not call us for results unless an unusual amount of time (over one week) has passed or you have been instructed to do so. Lab results are also available on the Patient Portal.



Billing

What you can expect from us: We accept many of the insurance plans currently offered in the Chicago and Northwest areas. As a courtesy to you, Des Plaines Family Medicine will:

  • Submit claims on your behalf directly to your insurance carrier for appropriate payment.
  • Provide you with information regarding balances owed to us once all insurance payments have been made.
  • Provide you with an account statement each month that your account has a balance.

What we expect from you: Des Plaines Family Medicine expects the following from its patients upon provision of services:

  • Patients must provide us with their current insurance cards and a photo ID.
  • Patients will be responsible for any co-payments, deductibles or non-covered services at the time of service. This includes amounts denied or not covered by the patient’s insurance plan.
  • Medicare patients may be asked to sign a waiver for non-covered services at the time services are provided allowing us to bill you directly for these non-covered services.
  • Self-paying patients are expected to make payment arrangements at the time services are provided.
  • Patient balances are to be paid in a timely manner. Balances that have not been paid after three monthly statements will be turned over to our collection agency.
  • A fee of $25 will be charged to the patient’s account for any returned checks.
  • All disability forms, medical records, return to work forms, etc must be prepaid before they are mailed out.

Payment and Financing Options: Des Plaines Family Medicine offers the following payment and financial arrangements to facilitate the handling of patient financial obligations:

  • We accept cash, check, Visa, Mastercard, Discover and American Express for patient accounts. Medical records must be paid for with cash or check only.
  • The billing department can arrange a budget plan that requires a monthly payment. Please call our Billing Office at Health Care Data Services, 866-886-4392. Sandee will be happy to assist you.

Managed Care/HMO
We participate in a number of HMO Insurance Plans. Most HMO plans place specific restrictions on their coverage. As we cannot always remember who your insurance carrier is, PLEASE INFORM OUR STAFF WHENEVER YOU CALL OR COME TO THE OFFICE IF YOU ARE COVERED BY SUCH A PLAN OR IF YOU HAVE CHANGED PLANS. This will enable us to comply with the rules of your plan and refer you to the appropriate hospital, emergency facility, laboratory or specialists that are covered by your plan. IF YOU DO NOT INFORM US, YOU WILL BE RESPONSIBLE FOR FEES NOT COVERED BY YOUR PLAN. Please familiarize yourself with your HMO plan and its rules, benefits and restrictions. If you change from one HMO to another, please contact our office immediately so that we can update your registration.
