About our office

As a small private physician’s practice, Dr. Szachowicz and his staff are dedicated to providing quality, professional and personalized care to the patients we serve. All patients are treated with respect and given the individual attention they deserve. Dr. Szachowicz is a board certified physician in Family Medicine, and has been in private practice for over 20 years in Des Plaines, IL. He has admitting privileges at Advocate Lutheran General Hospital, Northwest Community Hospital and Holy Family Medical Center. As a family medicine physician, his practice includes patients from infants to senior citizens and encompasses not only health maintenance but also chronic disease management. Dr. Szachowicz also enjoys participation in the education of our next generation of quality physicians and serves as a clinical preceptor for several area medical schools.

Do you accept walk-ins?

We do accept walk-ins, although we always prefer that you have an appointment so that we can have your information available and better manage patient flow. Same day appointments are generally possible.

What type of payments do you accept?

We accept cash, check, Visa, Mastercard, Discover and American Express for office visits or to make a payment on your account. If you are requesting your medical records to be transferred to another physician, we ask you to submit either cash or check payment only along with your signed authorization form.

Do you charge for missed appointments or no shows?

We do not charge for missed appointments or no shows at the present time, however our staff makes every effort to telephone reminder calls 1-2 days in advance of your appointment. If you are unable to keep an appointment, we ask for the courtesy of a phone call so that we can make your appointment available to someone else.

What is your medication refill policy?

Because we transmit our prescriptions electronically, the most efficient method is to have your pharmacy request a refill electronically from us. The office will not refill prescriptions via telephone request over the weekend.

Where should I get my blood work done?

Our office is equipped to perform some basic chemistry and urine screenings, depending on your insurance coverage. We also send out testing to several reference labs, or you can go on your own to various sites for testing.

Do you accept my insurance?

Our office participates with most major insurance companies; however it is always a good idea to verify our participation with your insurance company prior to your visit. Please bring your current insurance card with you to each appointment.

Is the office wheelchair accessible?

Our building and office is wheelchair accessible. In addition, there is transportation assistance available through several agencies. You may call our office for more information.

Is there parking available?

Ample parking is available in the immediate vicinity.
